Fees for the 2020-21 US Medical Residency Application Cycle

Prepared residency applicants know there are many costs and fees along the road to residency. Early awareness of these fees will help those applying for US medical residency to better organize their financial resources and budget accordingly. Below are all of the fees and costs associated with the US Medical Residency Application process from start to finish, and everything in between.

USMLE (For IMGs)

Exams

  • USMLE Step 1 = $965 + International Test Delivery Surcharge, if testing outside the US and Canada
  • USMLE Step 2 CK = $965 + International Test Delivery Surcharge, if testing outside the US and Canada
  • USMLE Step 2 CS = $1,600

Additional Services

  • Extension of Eligibility = $90 per exam
  • Step 1/Step 2 CK Testing Region Change = $85 per region change
  • Step 1/Step 2 CK Rescheduling Fee = $0 – $604 (depending on exam, testing region, and date of cancellation)
  • Step 2 CS Rescheduling Fee = $0 – $1,300 (depending on date of cancellation)
  • Score Recheck (Step 1/Step 2 CK/Step 2 CS) = $80 per exam

ECFMG 

ECFMG Services

  • ERAS Token = $130
  • Application for ECFMG Certification = $145
  • Application for J-1 Sponsorship = $350  (Plus $220 fee to the Department of Homeland Security, required for initial applicants of J-1 sponsorship)
  • Medical School Transcript Translation = $250 (if needed)
  • CVS Verification to State Medical Licensing Authority = $55

ECFMG + USMLE

  • USMLE Transcript (Paper Copy) = $70 per request (up to 10)
  • USMLE Transcript (Electronic) = $80 (Once per ERAS season)

AAMC/ERAS 

Programs Per Specialty

  • Up to 10 = $99
  • 11 – 20 = $15 each
  • 21 – 30 = $19 each
  • 31 or more = $26 each

For example, in one specialty:

  • 100 programs = $2,259
  • 200 programs = $4,859

Or, if you want to split up your application

  • 100 programs in one specialty + 100 programs in a different specialty = $4,518

**Note: AAMC/ERAS takes Visa and Mastercard only. All fees are non-refundable, therefore, you will want to research programs carefully.

  • Registration Fee =  $85 (first 20 programs on primary rank list)
  • Late Fee =  $50 (after Nov. 30)
  • Couples = $35 additional per partner
  • Additional Programs = $30 per program
